Hospital admission procedures

When being admitted to the hospital for treatment, customers need to prepare:

• Medical examination book or hospital admission letter from the doctor (if any).
• ID card, Health insurance card (if any).
• Kết quả X-Quang, siêu âm và/hoặc các xét nghiệm/chẩn đoán có liên quan. Đối với trẻ em: Mang theo đồ chơi yêu thích, sữa uống, bình sữa và các nhu cầu chế độ ăn uống đặc biệt (nếu có).
• Phí tạm ứng nhập viện (theo thông báo của nhân viên phụ trách nhập viện). Hạn chế mang theo các tài sản cá nhân có giá trị cao, nếu có, xin vui lòng liên hệ nhân viên để được hướng dẫn bảo quản tại két sắt an toàn. Trước khi nhập viện, Quý khách và người nhà sẽ được nhân viên phụ trách hướng dẫn cụ thể các thủ tục và giải thích rõ ràng về quy định, chi phí cũng như các vấn đề liên quan. Sau khi làm các thủ tục nhập viện, Quý khách được đưa đến phòng lưu trú trong quá trình điều trị. Tất cả các tầng điều trị đều có đội ngũ y tá, điều dưỡng túc trực 24/24 hỗ trợ, tư vấn Quý khách trong suốt quá trình điều trị tại bệnh viện.

Hospital discharge procedures

• Discharge procedures will be performed in the morning every day.
• You will receive medical documents when discharged from the hospital, including: Hospital discharge medical report, test results, image diagnosis results, expense list and payment receipt, hospital discharge papers, Birth certificate (in case of childbirth), prescription (if any).
• If you want to take medicine back from the hospital according to your discharge prescription, please notify the nurse to complete the procedure. Medicine costs will be included in your hospital discharge payment.
• In case of birth at the hospital: You will be guided by the administrative staff of the Obstetrics Department to fill in the necessary information on the birth certificate. A birth certificate will be issued when you leave the hospital. In case the contact person to get the Birth Certificate is not the mother, the authorized person needs to bring the mother's ID card and documents to complete the procedure.
• In case your hospital fee has been confirmed by the insurance company, you still need to go to the cashier to sign to confirm the payment invoice and complete the hospital discharge procedure.

When discharged from the hospital, please note:

• A doctor's prescription for home maintenance treatment along with specific instructions on the drugs and how to use them;
• Hospitalization certificate signed by doctor;
• Results of diagnostic imaging (X-ray, CT scan), blood tests performed during treatment;
• Inform details of the treatment process if necessary;
• Appointment card for follow-up examination if prescribed by the doctor;
• Personal belongings and items to bring with you when entering the hospital.
